IMPORTANT INSTRUCTIONS – PLEASE READ!!

Welcome to the 2023-24 school year! In this packet you will find all the back to school information you need, as well as information and donation forms for the many great parent organizations, booster clubs, and student activity groups that we are fortunate to have at Peninsula High.
 
Please take the time to go through this packet thoroughly. It is best completed in one sitting on a computer, not phone.
 
IMPORTANT INFO FOR COMPLETING THIS PACKET:

*NAVIGATION. The contents of this packet are listed and labelled below. Select ‘View Packet’ to begin and proceed through the forms using the ‘Next’ button. You can scroll to the bottom of any sections that do not pertain to you or your students and use the ‘Skip Page’ button. You can return to any skipped pages later through the list on the bottom of this page. NOTE THAT YOU MUST COMPLETE THE FINAL PAGE IN THE LIST, ‘REVIEW AND SUBMIT’ FOR ANY MEMBERPLANET PAYMENTS TO COMPLETE.

Note that Link Crew and ASB are examples of this - purchase online through the provided Webstore link by Monday August 14th at 9am PT in order to receive the pre-order discount AND reduce your students' wait time at Photo/ID Day.
 
*PRINTABLE FORMS. Forms for some organizations will need to be printed and submitted, and a link is provided above these fliers that will link to a printable version that opens in another browser window. In most cases you may also click on a form to be taken to a printable version.
 
*PAYMENTS. Webstore and Memberplanet payments will be processed separately. YOU MUST COMPLETE THE FINAL PAGE IN THE LIST BELOW, ‘REVIEW AND SUBMIT’ FOR ANY MEMBERPLANET PAYMENTS TO COMPLETE. Credit cards, debit cards, or e-checks are the preferred method of payment for Memberplanet forms, and these transactions will be listed as ‘Memberplanet LLC’ or ‘MP’ on your statement. You will receive a combined email receipt for these payments. Webstore payments must be completed within the Webstore, and you can always reference these payments later through your Webstore account info. Some organizations offer printable forms and accept check payments – follow instructions on each form for how/where to return these.
