Pharmacist: (1 Contact Hour of CE) - Identify factors which put patients at risk for medication-related injury and related warning signals
- Apply age-related changes that can affect pharmacotherapy to identify areas of therapeutic concern
- Evaluate and compare strategies to mitigate medication-related injury in vulnerable populations

The New Jersey Society of Health-System Pharmacists (NJSHP) is accredited by the Accreditation Council For Pharmacy Education as a provider of continuing pharmacy education. Credit will only be awarded for individuals who successfully fulfill all requirements, including verified attendance, successful completion of the activity, and submission of the post-session meeting evaluation. Credit is uploaded to CPE Monitor within 45-days of the activity.
